Your record is yours

What is already online, free

MyWinterfield holds most of what people ask for, going back to 2019, at no charge and with no request form:

• Laboratory and imaging results, released as soon as they are verified rather than held for a call.
• Visit notes and discharge summaries.
• Your medication list, allergies, and immunization record.
• Problem list and diagnoses.
• Upcoming appointments and past visit history.
• A download of the whole thing, as a standard health record file your other providers can import.

Results appear as soon as they are final. That means you may see a result before your clinician has called about it. We do that deliberately, and the alternative — holding your own result back from you — is worse. If a result worries you, message your care team through the portal or call the nurse line on (619) 555-0147.

Requesting a copy

For anything not in the portal, for records before 2019, or for a copy sent directly to someone else, put in a written request to health information management.

Cost. Electronic copies are free. Paper is $0.10 a page after the first fifty, capped at $25 per request. Records sent to another treating provider are always free, and there is no charge at all if the request is for continuing care.

Records for someone else

• Your child under 12. A parent or guardian may request the full record with proof of relationship.
• Your child aged 12 to 17. Most of the record, but California law protects some categories — including reproductive, behavioral health, and substance use care — which the young person controls themselves.
• An adult who cannot request for themselves. You need a power of attorney for health care, a conservatorship order, or a signed authorization from them.
• Someone who has died. The executor or personal representative, with letters testamentary or an equivalent court document.

If something in it is wrong

You have the right to have an error corrected, and to have your own statement added to the record if we disagree with you about it. Write to records@winterfieldhealth.com saying which entry is wrong and what it should say. We will respond within sixty days.

A correction does not delete the original entry. Health records are append-only by design, so the original stays visible with the correction attached to it, which is what makes the history trustworthy.

For other providers and their systems

Treating providers can request records directly, without going through the patient, for continuing care. Call (619) 555-0173 or send the request on your organization's letterhead.

Winterfield exposes a standard FHIR R4 interface for connected systems, covering patients, encounters, conditions, observations, medications, and practitioners. Integration questions go to the same number. There is a diagnostics console for partners already connected to it.
